To lessen the chances of any firmware-related glitch, make sure that you PS4 is always updated. In some rare instances, a coding issue with the firmware can cause audio to fail. Both of them can be accessed under the Settings menu. Make sure that you check the settings for Audio Devices and Audio Output Settings. There are two sets of settings that you need to check to ensure that your PS4 can forward audio to the TV or to the controller (if you’re using a controller-connected headset).
